Clincor is documentation software for licensed clinicians. Please read this before relying on anything it produces.
The clinician reviews and signs. Every note Clincor writes is a draft until the treating clinician reads it, corrects it and finalizes it. Finalizing is the clinician's attestation that the note is accurate and complete. Clincor marks lines that came from the practice's usual wording rather than from the dictation so they can be checked before signing.
Drafts are not medical advice. Suggested plans for the next visit are drafts built from the practice's own notes and the recorded measurements, using plain rules that are shown with each suggestion. They are labeled "For your approval" and have no effect until the clinician approves or changes them.
Clincor does not diagnose. It does not examine patients, interpret imaging or tests, establish diagnoses, or decide treatment. Where it notices words that often matter for screening (for example new numbness, weakness, night pain, or bowel and bladder changes), it points to them for the clinician's attention. It does not decide what they mean.
Measurements are read from what was said or written. Values such as range of motion, pain scores, vital signs and strength grades are extracted from the dictation or note text. The clinician is responsible for confirming each value before finalizing. Trend arrows compare recorded numbers only; the meaningful-change margins Clincor uses are general guides, not clinical standards for any individual patient.
Emergencies. Clincor is not a monitoring or alerting system and must never be relied on to detect or respond to an emergency.
